The cardiovascular perfusionist operates the heart-lung machine during cardiac surgery, temporarily replacing the function of the heart and lungs so the surgeon can operate on a stopped heart. They control blood flow, oxygenation, temperature, anticoagulation and heart protection, and manage support techniques such as ECMO. It is a role of maximum responsibility: while on bypass, the patient's life is literally in their hands.

Their main duties

During a bypass operation, the perfusionist:

Beyond the cardiac OR

The perfusionist's work goes beyond scheduled heart surgery. They also handle ECMO (life support for severe cardiac or respiratory failure), transplants, ventricular assist devices, blood salvage and, in some centres, regional perfusion procedures. On ECMO, see ECMO explained.

A typical day

The day usually starts early, reviewing the surgical schedule and preparing equipment. Before each case the perfusionist sets up and checks the circuit, coordinates with surgeon and anaesthetist, and verifies safety step by step. During surgery their attention is total. Scheduled cases are joined by on-call duty, because cardiac emergencies, transplants and ECMO have no timetable.

What kind of person it takes

It suits people who are meticulous, calm under pressure and highly focused. Communication with the surgical team is key: the perfusionist works as part of a mechanism where every second counts. Technical training is demanding, but temperament and teamwork matter just as much.

How to get there

If this appeals to you, the path is in how to become a perfusionist, and if you come from nursing, in from nurse to perfusionist.
